Medical History Report

Office Tour
When you visit us for the first time, we want you to feel welcomed. That’s why, just as you would when you visit someone’s home, you’ll take a tour of our office. We’ll let you see the standard examination rooms and equipment we use.
Discuss Your Concerns
The next step of the process entails speaking with a wellness chiropractor from our practice about your specific concerns. We’ll want to know if any pain or discomfort you have and when it’s more prevalent.
If you don’t have any specific concerns and are looking for our preventative care, we’ll want to know about your goals or if you’re interested in improving your health overall.
Determine if Imaging Is Necessary
In most cases, we’ll conduct X-rays to visit your skeletal structure and assess for any abnormalities, such as spinal subluxations.
Based on the results, we can figure out the appropriate treatment for you, like the target area of your adjustment.
Conduct a Physical Assessment
Your first visit often consists of a physical examination, during which time, a wellness chiropractor will assess your range of motion and examine an injured area for bruising, swelling, or other signs of trauma.
Create a Personalized Care Plan for You
From our findings, a wellness chiropractor from our team will create a care plan tailored to your specific needs. At your visit, you’ll be told what your treatment will entail as well as the frequency of it and an anticipated timeline. This plan may change as you continue care with us.
